To verify your credential to another state board, submit $10.00 for each verification. Example: $10.00 is required for each credential you are verifying; $10.00 is required for each state where a verification is to be sent. Make check payable to: Wisconsin Department of Regulation and Licensing.

- Daytime Phone Number
- Mailing Address where verification is to be sent
- If you are requesting verification of exam scores only, please indicate the approximate date you took the exam(s).
